I'm frequently getting this.  I'm not sure what I'm supposed to do because hitting refresh doesn't change anything.  Opening it in a new tab or window doesn't help either because I still get the same message.

Do you mean "Profile Refresh Needed"?

If so, then look for those options on the "Online" menu. These are options that update the local database to reflect the latest online database contents.

F5 (Refresh Collections List) is different. This updates the UI to reflect the latest contents of the Local database.

Yeah, it just means you haven't downloaded any updates to your collection for a while. It's just letting you know there may be new information you haven't downloaded.
